Disney Itinerary 2020




Dear Band family,


I am very much looking forward to accompanying the Roslyn High School Marching Band on the Disney Trip this year. In anticipation of a completely magical Disney experience, I am asking that you let me know any medical concerns or needs that your child may have before the trip.


Any student who will require medication during the trip needs both a provider order and parent permission. Parents, whose students will require medication for the trip, which they do not require on a daily basis at school, must obtain physician’s orders and provide the medication. Please complete the attached form and return it to Mr. Mauriello by 2/10/2020. Medications can be delivered to me, in their original container with the orders attached, at the drop off (6:30pm) on Saturday, 2/15/2020.


As a reminder, students with life-threatening allergies or asthma should carry their Epi-Pens and/or inhalers in their backpacks and have them available at all times. If you do not have an updated Self-Medication Form on file, I will be contacting you within the next 24 hours.


On a final note, parents are often surprised to discover that I am not permitted to administer Advil/Tylenol/Motrin, Claritin/Zyrtec/Benadryl, Lomotil/Imodium and other common over-the-counter medications without prior written consent. Therefore, if you anticipate your child carrying these OTC medications, a Self-Medication Form is required. Also, medication that must be kept cold during the bus trip need to be in a container with freezer packing. If you anticipate the need for any of these medications to be administered by me, a Medication Administration Form must be completed. Each of these forms require the signature of both the parent and the physician. Forms are attached.


If you have ANY concerns or insights that you wish to share with me, I encourage you to call or email me at the number listed below. In addition, I am providing my cell phone number for any concerns that may develop during the course of the trip. Please feel free to share this number with your child so that they can reach me in the event of an emergency.
